By filling up this form, I agree that my data can be used for source and contact tracing by the GGD in the event of an infection. I agree that my full name, e-mail address and telephone number will be shared in the event of an infection.

By agreeing, I also agree that:
I have had 0 of the following symptoms in the past 10 days: sneezing, coughing, >38 °C fever, shortness of breath.
I do not have a housemate with such symptoms.
If I had (been tested positive for) the coronavirus, it was not during the past 10 days.
If I have a housemate with the coronavirus, I have not made contact with them in the past 14 days.

If I do have symptoms prior to the social, I will inform ASDV Bonaparte that I cannot attend.

Door dit formulier in te vullen, ga ik ermee akkoord dat mijn gegevens gebruikt kunnen worden voor bron- en contactonderzoek door de GGD bij een besmetting. Ik ga ermee akkoord dat mijn volledige naam, e-mailadres en telefoonnummer worden gedeeld bij een besmetting.

Door akkoord te gaan, ga ik ook akkoord dat:
Ik heb de afgelopen 10 dagen 0 van de volgende symptomen gehad: niezen, hoesten, >38 °C koorts, benauwdheid.
Ik heb geen huisgenoot met dergelijke symptomen.
Als ik het coronavirus had (positief getest was), was dat niet in de afgelopen 10 dagen.
Als ik een huisgenoot heb met het coronavirus, heb ik de afgelopen 14 dagen geen contact met hen gehad.

Als ik symptomen heb voorafgaand aan de social, zal ik ASDV Bonaparte laten weten dat ik niet zal bijwonen.
